Nutrition Referrals

Thank you for your interest in referring a patient to Maria Taylor Nutrition.

I am accepting new patients (adults and teens 16+) at this time.

Referrals may be submitted in one of the following ways:

  • Secure Fax (Preferred for Clinical Details)
    Send fax referrals, including clinical information and protected health information (PHI), to: 📠 206-385-7380

  • General Referral Inquiry (Non-PHI):
    Use the form below to submit a general referral inquiry that does not include protected health information.

After the referral is received, the patient or caregiver will be contacted to complete intake, generally within 2-3 business days.

Please do not submit PHI via this form. If referral documents include medical records or clinical details, submit via secure fax at 206-385-7380 and note you are referring to Maria Taylor
